Admissions Checklist
Having the required records ready helps our admissions team review your child's information and keep enrollment moving.
Admissions Needs Checklist
The following items must be submitted:
- Completed A-FITS admissions application in its entirety.
- Copy of student's birth certificate.
- Most recent copy of student's IEP.
- Most recent copy of student's MET.
- If you have applied for or currently have ESA, proof of the ESA scholarship or a copy of the confirmation showing that you have applied.
- Proof of residency — a bill or other accepted document showing the parent or guardian's name and physical address.
- Immunization records.
- Photocopy of parent or guardian photo identification, such as a driver's license.
Submitting Educational Records
Your child's IEP, MET, and other educational records help A-FITS understand previous evaluations, academic history, supports, accommodations, therapy, behavior, and areas of continued need.
A-FITS reviews those records for history and context. If the student enrolls, A-FITS develops its own Individualized Learning Plan (ILP) based on our program and our understanding of the student.
ESA Documentation
If your family is using or applying for Arizona Empowerment Scholarship Account funding, please provide the applicable ESA documentation available to you during enrollment.
ESA documentation and A-FITS admissions documentation are separate processes, so our team will let you know if additional school records are still needed.
